Referral Process for Doctors

1. Download Referral Form
2. Acquire prior authorization* from patient’s insurance company (where applicable). If you are unable or are having trouble acquiring prior authorization from insurances, we can help.  Call us at (808) 456-7378.
3. Fill out referral form with patient information and fax to:
   (808) 483-8822 (Pearl City and Honolulu)
   (808) 969-8189 (Hilo and Kona)
4. Once we have the referral form and insurance has authorized a sleep study, we will call the patient to schedule their appointment as well as remind the patient three (3) days before the study and the day of the study.
5. You will receive the sleep study report 5-10 business days after the sleep study is performed
